Introduction

Metformin, a medication used in the treatment of type 2 diabetes mellitus (T2DM), is a widely recognized medication for its efficacy in controlling blood glucose (sugar) levels. Metformin is a selective glucose uptake inhibitor that is primarily prescribed for type 2 diabetes management. Metformin is also used to treat the symptoms of diabetic ketoacidosis and to prevent or reduce the symptoms associated with diabetic nephropathy (DPN) in individuals with post-traumatic stress disorder (PTSD) and to manage chronic kidney disease (CDK) and hypertension. The World Health Organization (WHO) estimates that approximately 80 million people worldwide suffer from diabetes, affecting about 2.2 billion people worldwide. Metformin is considered the cornerstone of therapy for treating T2DM and is associated with a significant improvement in diabetic kidney function. The primary efficacy and safety of metformin in the treatment of diabetic nephropathy are not fully understood, but its role in diabetes management has been established.

The primary aim of metformin therapy for diabetes is to improve glycaemic control by maintaining or increasing glucose production and maintaining peripheral glucose output. The drug has been shown to improve the levels of circulating glucose (glucose) in the blood, but this has not been evaluated in the treatment of diabetic nephropathy. In the absence of adequate glucose control, metformin has been recommended in the management of diabetic nephropathy. Metformin is approved for the treatment of type 2 diabetes mellitus and is also used to treat the symptoms of diabetic kidney failure. In addition, metformin is indicated for the treatment of the symptoms of diabetic nephropathy and to reduce the severity of diabetic nephropathy in patients with chronic kidney disease or diabetic nephropathy associated with advanced renal failure.

Several studies have investigated the efficacy and safety of metformin for the treatment of diabetic nephropathy in patients with post-traumatic stress disorder (PTSD). In one of these studies, the efficacy of metformin in reducing the severity of the symptoms of diabetes mellitus was evaluated in combination with the anti-psychotic medication benzodiazepines. In another study, the efficacy of metformin in reducing the severity of the symptoms of T2DM in patients with post-traumatic stress disorder (PTSD) was evaluated in combination with the benzodiazepine antidepressant (bZD). In a randomized, double-blind, placebo-controlled trial, we examined the efficacy and safety of metformin for the treatment of diabetic nephropathy.

The present randomized, double-blind, crossover, multicenter study, which included 16 patients with type 2 diabetes mellitus, showed that metformin significantly reduced the severity of diabetic nephropathy compared to placebo. Moreover, patients receiving metformin showed a statistically significant improvement in the number of patients with a glomerular filtration rate (eGFR) below 15 mL/min/1.73 m2 (median change from baseline, 0.1 mL/min/1.73 m2, p<0.001). Thus, metformin is a potentially effective treatment option for the treatment of diabetic nephropathy.

Mechanism of Action

The action of metformin on glucose metabolism is based on the inhibition of an enzyme, glucose-dependent protein synthesis (GDS), and the activation of insulin-dependent protein synthesis (IRP). The main mechanism of action of metformin is to inhibit the hepatic synthesis of glucose from glucose by the liver. The inhibition of this hepatic enzyme leads to increased levels of circulating glucose in the blood, which may lead to the decrease in glucose production. Metformin also prevents the conversion of glucose to other compounds, thereby decreasing the amount of glucose produced by the liver. Metformin has the potential to exert its therapeutic effect by decreasing the amount of circulating glucose produced by the liver, thus promoting normal glucose metabolism.

In vitro studies have shown that metformin has a specific affinity for the liver protein binding proteins, glucose-6-phosphate dehydrogenase (G6PD) and glucose-dependent protein kinase (GSK) 1. In addition, the inhibition of these enzymes by metformin has been reported to be dose-dependent, with a maximum concentration of metformin present in the blood at least once per day. Therefore, metformin has the potential to be an effective option for the treatment of diabetes mellitus, particularly for patients with type 2 diabetes mellitus.

Faster absorption of metformin

Metformin is absorbed mostly by the liver and intestines. It is cleared mainly by the kidneys. The absorption of fda approved Metformin is not as fast as some other diabetes medicines. It takes around 2-3 hours to be fully cleared from the blood. The peak concentration of the drug in the urine is reached within 1-2 hours after taking the drug.

How to use metformin

The usual dose of metformin is one tablet per day. If there is no response to taking the tablet regularly, you may try taking it on an empty stomach or with food. Take it at about the same time(s) every day.

The maximum recommended dose of metformin for adults is one tablet per day.

There is no evidence of increased risk of suicidal thoughts and/or behaviors in children or adolescents. If metformin is to be taken during pregnancy or breastfeeding, please inform your doctor so that they can continue the use of the medicine.

How to handle metformin

Metformin should be used with extreme caution in children and adolescents with liver problems. If metformin is to be taken along with a reduced-calorie diet and exercise, please inform your doctor so that they can continue the metformin dose.

Metformin effects on sugar and sodium

If metformin is taken with sucralfate, please inform your doctor so that they can continue the metformin dose. Sucralfate can cause diarrhoea and indigestion in some children. If diarrhoea, indigestion and indigestionate are severe or don’t go away, please consult your doctor.

Metformin and alcohol
